Order death certificates online through VitalChek®, the only vendor authorized by the Florida Department of Health.
If cause of death is requested, it must be an immediate family member of legal age or a legal representative representing immediate family member or estate of deceased. (Florida Statute 382.025, F.S.). Legal representative must provide legal documentation.
Vital Statistics Services are available Monday – Friday from 8 a.m. – 4 p.m.
