Disease Reporting Information
Health care and laboratory providers are required to report conditions of public health importance to the Florida Department of Heath.
Communicable diseases or conditions are transmitted from person to person. This places the entire community at risk. When a communicable disease is suspected, the Florida Department of Health is the first agency to be contacted.
The Epidemiology Department at the Florida Department of Health in Lafayette County investigates and monitors communicable diseases in the county. Our epidemiology nurse is trained to assess, evaluate and monitor communicable disease reports to help prevent epidemics. This protects individuals, families, and the community from disease outbreaks.
A nurse is available to take reports of communicable diseases Monday through Friday and an after-hours nurse is available to take reports nights and weekends. Please call 386-294-1321.
