Vital Statistics Services are available: 8:00am - 4:00pm Monday through Friday.


If cause of death is requested, must be an immediate family member of legal age or a legal representative representing immediate family member or estate of deceased. (Florida Statute 382.025, F.S.) Legal representative must provide legal documentation.

  • Applicants must have a picture ID
  • Years available: 2007 to the present (statewide)
  • Fees: $7.00 each certified copy
  • Payment in cash or check
